Retrograt İntrarenal Cerrahi Geçiren Hastalarda Nöromuskuler Ajan Kullanımının Postoperatif Titreme Üzerine Etkisi: Randomize Kontrollü Klinik Çalışma

GİRİŞ ve AMAÇ: Titreme ve aktif hareket bilinci açık ve hareket edebilen hastalarda ısı üretimi ve ısınma için çok etkin bir mekanizmadır. Bu çalışma genel anestezi sırasında kas gevşetici (KG) kullanılan erişkinlerde intraoperatif hipotermi ve buna bağlı postoperatif titreme olabileceği hipotezini test etmeyi amaçlamıştır.
YÖNTEM ve GEREÇLER: Retrograd intrarenal cerrahi (RIRS) geçirecek 80 hasta rastgele kas gevşetici (KG) kullanılan (Grup R, N=40), KG kullanılmayan (Grup K, N=40) olarak iki gruba ayrıldı. Tüm hastalara havayolu kontrolü için Laringeal Maske (LMA) kullanıldı ve KG verilmesi dışında, standart bir anestezi protokolü uygulandı. Cilt ve iç ısıların monitörizasyonlarından elde edilen veriler ve titreme muayeneleri anestezi indüksiyonundan önce (t0), KG verildiğinde (t1), intraoperatif 15 (t2), 30 (t3), ve 60 (t4) ve postoperatif 0 (t5), 10 (t6), 15 (t7), 30 (t8), 60 (t9) dakikalarda kaydedildi.
BULGULAR: Grup R’de t0, t1, t2, t3 ve t5 cilt ısısı Grup K’dan anlamlı derecede yüksek iken iç ısılar bakımından gruplar arasında fark yoktu. (t5, t6 ve t7'de titreme olanların oranı Grup R’de Grup K’dan anlamlı derecede fazla idi (p<0,05). Her iki grupta da cilt ısısında t0’a göre istatistiksel olarak anlamlı fark görülmektedir. Buna göre K grubunda t7,8,9’da t0’a göre anlamlı fark saptanırken R grubunda t1’de farklılık saptanmıştır. Grup R’de iç ısı t3’de t2’ye ve Grup K’da t3 de t1, ve t2’ye göre anlamlı düşüktü.
TARTIŞMA ve SONUÇ: Postoperatif titreme ve buna neden olan mekanizmaların anlaşılması önemli bir konudur. Hipotermi üzerinden sinir kas gevşetici kullanımı ile muhtemel bir ilişki ise intraoperatif ısı yönetimi ve respiratuar ve kardiyak rezervi düşük hastalar açısından araştırılmaya değer. Bu çalışmadan elde edilen veriler, KG kullanımı ile intraoperatif hipotermi arasında bir ilişkiyi desteklemezken, postoperatif titreme ile bir ilişkiyi açıkça göstermiştir. Bununla beraber, vücut sıcaklığına kas gevşeticilerin muhtemel etkileri ve erişkinlerdeki periferik savunma mekanizmalarının ve belki ilave olarak santral mekanizmaların çalışılmasına gereksinim vardır.

The Effect of Neuromuscular Agent on Postoperative Shivering in Patients Undergoing Retrograde Intrarenal Surgery: A Randomized Controlled Clinical Trial

INTRODUCTION: It is a very effective mechanism for heat generation and warming in patients with open and movable consciousness. This study aimed to test the hypothesis that intraoperative hypothermia and postoperative shivering may be present in adults using neuromuscular blocker (NMB) during general anesthesia.
METHODS: Eighty patients who underwent retrograde intrarenal surgery (RIRS) were randomly divided into two groups to receive either neuromuscular blocking agent (NMB) or not [Group R (NMB+), N = 40; Group K (control), N = 40)]. Laryngeal mask (LMA) was used for airway control in all patients and a standard anesthetic protocol was performed except for NMB administration. Data obtained from the monitoring of skin and internal temperatures and shivering tests were performed prior to anesthesia induction (t0), when given NMB (t1), intraoperative 15 (t2), 30 (t3), and 60 (t4) and postoperative 0 (t5), 10 (t6), 15 (T7), 30 (t8), 60 (t9) minutes.
RESULTS: In group R, t0, t1, t2, t3 and t5 skin temperature were significantly higher than group K, but there was no difference between the groups in terms of internal temperatures. (t5, t6 and t7 in the proportion of those who have tremors in group R was significantly higher than group K (CI-Square p <0.05). In both groups, there is a statistically significant difference in skin temperature compared to t0. According to this, there was a significant difference in t7,8,9 in Group K and t1 in group R according to t0. In Group R, the internal temperature was significantly lower in t3 than in t2, and t3 in group K was significantly lower than t1 and t2.
DISCUSSION AND CONCLUSION: An understanding of postoperative shivering and the mechanisms that cause it is an important issue. A possible association with the use of neuromuscular blocker over hypothermia is worth investigating for patients with intraoperative heat management and low respiratory and cardiac reserve. The data obtained from this study did not support a relationship between NMB use and intraoperative hypothermia, but clearly showed a relationship with postoperative shivering. However, there is a need to study the possible effects of neuromuscular blockers on body temperature and peripheral defense mechanisms in adults, and perhaps also central mechanisms.
